Summary: Cares
for adults in their homes as directed by the client care plan by performing the
following duties. The adults may
be seniors and/or hospice, transitional, disabled, injured,
or ill clients.
Essential Duties and
Responsibilities: include the activities of daily living (ADLs),
household and other supportive
services, medication services and delegated special tasks of
nursing care. Other duties may be assigned.
Assists the client with mobility and movement
(ambulation, transfers, and exercise). Turns and repositions bedfast clients,
alone or with assistance, to prevent bedsores.
Assists the client with bathing, dressing and personal hygiene.
Assists the client with toileting and perineal care.
Prepares and serves food for the client.
Provides medications services as directed by the care plan.
Cleans, dusts, mops, and vacuums the client's home. (light housekeeping)
Washes, dries, and puts away dishes.
Puts out and brings in the garbage.
Makes the bed and changes the bed linens.
Washes, dries, folds, and puts away client's laundry.
Runs errands and goes shopping with or without the client.
Transports client.
Provides companionship and activities.
Cares for client's pets.
Maintains records of services performed and of apparent emotional, mental and physical condition of client.
Takes and records temperature, blood pressure, pulse, and respiration rates and food and fluid intake and output.
Responds to emergency situation in accordance with established policies and procedures.
Observes safety and security policies and procedures.

Qualifications:
To perform this job successfully, an individual must be able to perform each
essential duty satisfactorily.

Education and/or experience: High school diploma or general education
degree (GED); and six months experience supervised
by a registered nurse in a facility setting; or equivalent
combination of education and experience.
Language
Skills: Ability to read and interpret documents such as safety rules,
operating and maintenance instructions, and
procedure manuals. Ability to write routine reports and
correspondence. Ability to effectively present information in
one-on-one and small group situations to clients and other
employees.
Mathematical Skills: Ability to add, subtract, multiply, and divide in
all units of measure, using whole numbers, common fractions,
and decimals. Ability to compute rate, ratio, and
percent.
Reasoning
Ability: Ability to apply common sense understanding to carry out
instructions furnished in written, oral, or
diagram form. Ability to deal with problems involving several

Certificates, Licenses, Registration:
An individual must have an Oregon Certified Nursing Assistant license.
An individual must have an Oregon or Washington driver's license with:
No more than three moving violations in the past 36 months.
No more than one chargeable accident in the past 36 months.
No major convictions (drunk, drug, or reckless driving) in the past seven years.
An individual must have or be able to obtain:
Heartsaver Adult CPR card.
First aid card.
An individual must have a reliable car with automobile insurance with liability limits of insurance equal to,
or greater than, those mandated by Oregon.
An individual must pass a pre-employment drug screening test and a criminal background investigations.
An individual must be willing to travel to clients' homes in Clackamas, Multnomah, and Washington counties.
An individual must be willing to work three to twelve hour shifts/visits.

Must have either CNA license or completion certificate from a CNA program and/or at least 6 months experience in a nurse supervised facility (AFH, RCF, ALF, nursing home, or hospital). Considerations may be made for those who are currently enrolled in a health care program.
Must be able to obtain First Aide and CPR certification
Uniforms must be either color coordinated scrubs or nurses smock with white pants.
